To create a new form, or edit an existing form, click on “log-in” at the top of the page.
Once logged-in, you can submit a CATS by clicking the Shortcuts button on the top menu bar and clicking on the "Create a new Assessment" link. To edit an existing form created earlier, click directly on the form. You can find existing forms by clicking the "Assessments by Division" button (left side) or by searching the CATS system in the top-right corner.
